How to Refer a Patient

The UAB Medicine Cystic Fibrosis Program currently is accepting new adult patients; referring physicians should call 205-975-0763. Our schedulers will ask for demographic and insurance information, then they can schedule an appointment in our clinic. Once the appointment is made, the referring office should fax the patient’s records to 205-975-2440.

Program Highlights

Our CF program is the only one in Alabama accredited by the Cystic Fibrosis Foundation and is closely connected to the foundation’s Therapeutics Development Network. Our CF care team developed an integrated practice unit, by which multiple providers can evaluate patients during a single clinic visit and deliver the latest, most advanced care. Adult and pediatric colleagues work closely together and with the CF community to deliver seamless care throughout life while developing rigorous quality improvement initiatives (through the CF Learning Network), with public transparency of outcomes.
