Opened on Jan. 15, 2019, this convenient, state-of-the-art medical complex offers comprehensive primary care services for adults and children, an adjacent Freestanding Emergency Department and a UAB Callahan Eye Hospital Clinic. The facility also offers specialty care services, for adults only.


The outpatient care provided at UAB Medicine Gardendale is backed by the knowledge and expertise of UAB, one of the nation’s leading academic medical centers. The full range of services provided by Gardendale Primary Care includes:

  • Adult and pediatric primary care (internal medicine)
  • Sports medicine
  • Travel medicine
  • On-site laboratory services
  • Comprehensive imaging services including echocardiogram (echo), electrocardiogram (EKG), digital radiology, mammography, magnetic resonance imaging (MRI), computed tomography (CT), and ultrasound technology

What to Bring with You
When you come to your appointment, please bring your insurance card, a photo ID, bottles of any prescription and over-the-counter medications you take, any vitamins and dietary supplements you are currently taking, and any important medical information from other health care providers.

Insurance & Payment Information
Your insurance co-payment is expected at the time of your visit. It is our policy to file insurance claims with Blue Cross/Blue Shield, Medicare, Medicaid, UAB VIVA, VIVA Health, and most other insurance providers. When you call for an appointment, we will verify your coverage.

Labs & Test Results
You will be notified of any normal test results either by telephone, mail, or at your next office visit. Any significant abnormal lab or test values will be communicated to you by your physician as soon as the results are obtained. Patients who have signed up for the myUABMedicine Patient Portal may view results of select lab work and other tests through their myUABMedicine account.

To request refills for any medications prescribed by UAB Medicine Gardendale, please call our office at 205.631.3452. Please have your pharmacy name and telephone number available to provide to our staff. Calls regarding medication refills should be made between 8 am and 3 pm. Please allow 24 hours for routine prescriptions to be filled. We suggest calling your pharmacy to make sure your medication is ready before visiting the pharmacy. Patients who have signed up for the myUABMedicine Patient Portal may request prescription refills through their myUABMedicine account.

New Patients
Please arrive 30 minutes before a new-patient appointment to allow for completion of registration paperwork.

After Hours and Emergencies
If you need to reach a physician after hours, call 205-934-3411 and ask for the physician on call. If this is a true medical emergency, please dial 911 for immediate assistance. For after-hours and weekend urgent care for non-life-threatening illnesses and injuries, UAB Medicine offers an Urgent Care Clinic located in downtown Birmingham. Please note that our clinic follows the Alabama Department of Health’s required vaccine schedule as part of the standard care for our pediatric patients. This ensures the safety and wellbeing of all patients and family members here at UAB Medicine Gardendale and is a REQUIRED component of the physician and patient relationship. The COVID-19 vaccination is not a required vaccine to maintain care at our facility.
