For a consultation or to refer a patient, please complete this form or contact the UAB Liver Transplant Program office at (205) 975-5691, toll-free at 1 (866) 305-5691, or via fax at 205-975-2298. We will contact your patient to schedule an appointment. In the meantime, please look for a follow-up fax from us confirming the referral.
For urgent referrals or inpatient transfers, please call UAB MIST (Medical Information Service via Telephone) at 1-800-UAB-MIST (1-800-822-6478) or email mist@uabmc.edu. UAB MIST is a toll-free, 24-hour service that connects physicians anywhere in the country with UAB Medicine specialists.

Program Highlights
UAB Medicine is home to the only liver transplant program in Alabama and has performed over 3,035 procedures since its launch in 1983. This volume has helped our program distinguish itself as one of the nation’s most experienced, with outcomes that consistently rank among the best in the United States, and we continue to overcome barriers to transplantation. We streamlined the transplant evaluation process and reduced travel burdens for patients who live far from UAB, and we provide comprehensive pre-transplant evaluations and post-transplant outpatient care at several satellite clinics in south Alabama and Tennessee.
