HOW TO REFER A PATIENT
For a consultation or to refer a patient, please complete this form or contact the UAB Advanced Heart Failure/Transplant Program office at (205) 934-3438 or via fax at (205) 975-9320. We will contact your patient to schedule an appointment. In the meantime, please look for a follow-up fax from us confirming the referral.
For urgent referrals or inpatient transfers, please call UAB MIST (Medical Information Service via Telephone) at 1-800-UAB-MIST
(1-800-822-6478) or email mist@uabmc.edu. UAB MIST is a toll-free, 24-hour service that connects physicians anywhere in the country with UAB Medicine specialists.
PROGRAM HIGHLIGHTS
The UAB Heart Transplant Program boasts one-year and three-year patient and graft survival rates that are better than the expected rates and U.S. averages. We are equally committed to treating heart failure through optimizing medical management and expanding the role of mechanical circulatory support devices, including LVADs, defibrillators, and pacemakers. UAB was among the first in the nation to implant a Berlin Heart, and we are Alabama’s only SynCardia Total Artificial Heart Certified Center. Our state-of-the-art, 23-bed Heart/Lung Transplant ICU cares for patients in the crucial first hours after surgery.
