What happens when I am discharged?

Planning for your hospital discharge starts as soon as you are admitted into the hospital. It is a vital part of your care here. Good planning for discharge ensures that your transition from the hospital will be smooth and that you will know what to expect. Care managers and medical social workers will work with you to make sure you understand the discharge instructions, including the drugs you may be prescribed and what kind of follow-up care you may need. They also will help make any arrangements needed if you are discharged to a rehabilitation or other care facility. When discharged from UAB, you will be given written instructions to read and follow. Be sure to discuss any concerns you may have about leaving the hospital with the hospital staff.

Important Things to Know Before You Go

We are committed to your care while you are in the hospital and once you are discharged. Make sure to complete the list below with your caregiver before you leave, to make sure that you are equipped with the tools and information you need to continue care.

  • Do you have discharge instructions and information?
  • Do you understand all of the discharge instructions? If not, ask questions!
  • What medications and what dosages will you be taking when you leave?
  • Who should you call if you have any follow-up questions?
  • Have you scheduled any follow-up appointments that you may need?

    • Location/physician
  • Are there any home environment changes or home care arrangements that need to be made?
  • Do you know who is picking you up when you are discharged?
  • Are there any other special instructions or dietary restrictions you will need to follow?
