UAB House Calls
UAB House Calls delivers personalized primary care services to frail, homebound adult patients who live within 30 miles of UAB Hospital, in their private residence or in a retirement community setting. We also offer emotional support and address social factors for our patients and their families and caregivers. We provide access to doctors, nurse practitioners, social workers, and other health care workers, and we can work with the patient’s existing primary care provider or serve in that role. House Calls is not a substitute for home health care or hospice, but we can help coordinate those services.
Why UAB
UAB Medicine has been offering home-based primary care services for over 15 years and is the only program of its kind in the Birmingham area. We serve patients with ongoing conditions as well as those who recently were discharged from the hospital. Our program can discuss your health goals and treatment options with you; assist with advance care planning; administer vaccines for flu, pneumonia, tetanus, shingles, and COVID-19; and order tests as needed. Our goal is to stabilize and improve our patients’ health by helping manage their conditions and medications and arranging for other needed services, all while reducing their emergency room visits and hospitalizations.
