UAB Huntsville Collaborative Care

UAB Medicine Huntsville’s Collaborative Care Clinic was founded as a health care collaboration between the UAB School of Medicine’s Huntsville campus and the University of Alabama in Huntsville (UAH) College of Nursing. Its mission is to deliver excellent care to individuals with acute and/or chronic illnesses and provide preventive health care services to patients of all ages, with a focus on continually improving treatment results and patient satisfaction.

As with all UAB Medicine Huntsville clinics, we are committed to teaching. Health care in the United States has become more difficult to navigate, so health care providers who learn to work together in collaborative teams are better able to provide a full range of health care services. Medical students and nurse practitioner students work in teams to help deliver excellent patient care, under the close supervision of faculty from both schools.

Our clinic is staffed by family nurse practitioners, who are nurses with advanced degrees and training in managing common health care problems. Nurse practitioner training also focuses heavily on teaching patients how to improve their overall health and stay healthy, through both individual and group education. We look forward to working with patients who already are familiar with the other UAB Medicine Huntsville clinics, as well as new patients seeking a health care home.
