UAB Hueytown Clinic

UAB Health Center-Hueytown is located at 3029 Allison Bonnett Memorial Dr., Hueytown, AL 35023. Our office hours are 8 a.m. to 5 p.m., Mondays through Fridays, except holidays. Our dedicated staff includes three physicians and a nurse practitioner, all specializing in family practice.

Appointments
You do not need a doctor’s referral to make an appointment. To schedule, reschedule, or cancel an appointment, please call (205) 497-4083 and follow the prompts. Please cancel with at least 24 hours in advance.

After Hours and Emergencies
In case of an after-hour emergency, call 497-4083 and follow the phone prompts to page a physician.

What to Bring with You
When you come to your appointment, please bring your insurance card, a photo ID, co-pay, bottles of any prescription and over-the-counter medications, vitamins, and dietary supplements that you are currently taking, and any pertinent medical information from other health care providers.

Special Instructions
Please arrive 30 minutes before a new-patient appointment to allow for completion of registration paperwork.

Payment Information
Your insurance co-pay is expected at the time of your visit. If you are a private-pay patient, full payment is expected upon completion of your appointment. Financial counselors are available to all patients to discuss any problems or special circumstances.
It is our policy to file insurance claims with Blue Cross/Blue Shield, Medicare, Medicaid, UAB VIVA, and VIVA Health, as well as most other insurance providers. When you call for an appointment, we will verify your coverage.

Labs/Test Results
Please allow 2 to 4 weeks from the time of any lab or other tests to be completely processed. You will be notified of any normal test results either by telephone, mail, or at your next office visit. Any important abnormal lab or test values will be relayed to you by your physician as soon as the results are obtained.

Prescriptions
To obtain a refill for any medications prescribed by this clinic, please call (205) 497-4083 and follow the phone prompts. Please have your pharmacy telephone number available to provide to our staff. Please save refills and non-urgent calls for our regular office hours. Please allow 24 hours for call-in prescriptions and 72 hours for written prescriptions

Patients with Limitations
Patients visiting the UAB Health Center-Hueytown -may take advantage of convenient parking in the ground-level lot adjacent to the center. The clinic is wheelchair accessible.
