General Clinic Information
The Diabetes Bridge Clinic is the first of its kind in the nation to serve hospitalized patients on an intense insulin regimen. The Diabetes Bridge Clinic helps people go from hospital to home with a continuum of diabetes care.
Often, the severity of the illness and strong medical treatments in the hospital can disrupt a patient’s glucose and diabetes control. These conditions can make it unsafe to return a patient to usual oral antidiabetic medications. Temporarily, the patient may need intensive insulin therapy.
Our Diabetes Bridge Clinic team follows these patients on a weekly basis for several weeks to help make sure their glucose and metabolic control is optimal for a faster recovery. With the diabetes under control, healing can take place with less likelihood of complications from the recent acute illness.
No other hospital United States offers this service for patients with diabetes.
The Clinic is located on the first floor of the Kirklin Clinic. The appointments are made directly by the doctor in charge of your care while hospitalized at UAB Hospital. If you need to make changes once you go home, or for additional information, call (205) 801-7450. In the event that you need to reschedule or cancel your appointment, we appreciate your cancellation at least 24 hours in advance.
After Hours and Emergencies
If you need to reach your physician after hours, please call UAB Paging at (205) 934-3411 and ask for the Endocrinologist on call. If this is a true medical emergency, please dial 911 for immediate assistance.
What to Bring with You
If you come to a follow up appointment, please bring your insurance card, all prescription and over-the-counter medications that you are currently taking, and any additional information that your referring physician has not already sent to our clinic. It is very important that you bring a log with all the blood-sugar readings that you have taken since you were discharged from the hospital.
Patients with Limitations
Patients visiting the Kirklin Clinic may take advantage of convenient parking in the deck adjacent to the Clinic. Located at 2100 6th Avenue South, the parking deck is connected to the second floor of the Kirklin Clinic by a weatherproof sky bridge. High-top van access is restricted to the first floor of the clinic parking deck. Therefore, patients arriving in high-top vans and requiring wheelchair assistance should be dropped off at the front door of the clinic before the driver parks the vehicle. Valet parking is available on Level 2 of the deck; wheelchairs and additional assistance are located here as well.
