General Clinic Information
We are located on the 5th floor of The Kirklin Clinic of UAB Hospital. Our office hours are Monday-Friday, 8:00 a.m.-5:00 p.m. Please call (205) 975-8615 with any questions or concerns.

Appointments
To schedule an appointment, please call (205) 975-8615 or 1-800-333-6543 (if outside of the Birmingham area). In the event that you need to reschedule or cancel your appointment, please call either of the numbers listed above. If possible, we appreciate your cancellation at least 24 hours in advance.

After Hours and Emergencies
If you need to reach a physician after hours, please call UAB Paging at (205) 934-3411 and ask them to page the Surgical Resident on call. If this is a true medical emergency, please dial 911 for immediate assistance.

What to Bring with You
When you come to your appointment, please bring your insurance card, a list of any prescription or over the counter medications that you are currently taking (including dosage amount), and any medical records or films related to your condition that have not already been sent to our office by your referring physician.

Special Instructions
 

  • If you are a patient in our Advanced Lung Disease and Transplant Clinic:

When you arrive at The Kirklin Clinic:
Following your lab work on the 2nd floor (Laboratory), please proceed to the 3rd floor for a Chest X-ray (Radiology) and then to the 4th floor for Pulmonary Function Tests (Pulmonary). Following these tests, please proceed to the CV Surgery Clinic on the 5th floor for your visit with the physician.
 

  • If you are a patient in our Advanced Heart Failure and Transplant Clinic:

When you arrive at The Kirklin Clinic:
Following your lab work on the 2nd floor (Laboratory), please proceed to the 3rd floor for a Chest X-ray (Radiology) and then to the 4th floor to have your Echocardiogram and EKG (Cardiographics). After these tests are completed, you will be directed to the Cardiology Clinic on the 4th floor for your visit with the physician.


Payment Information
Some insurance providers may require a referral or pre-certification before seeing a specialist of this nature. Please be sure to bring a copy of this referral or pre-certification with you for insurance/billing purposes. If you are unsure of what your insurance company requires, please contact them in advance of your appointment and they will be happy to answer any of your questions. Your co-pay is expected at the time of your visit; however, our financial counselors are available to discuss any problems or special circumstances. Our financial counselors may be reached Monday-Friday, 8:00 a.m.-5:00 p.m. at (205) 801-9910.

Labs/Test Results
Your physician will discuss with you how and when your lab results will be reported to you. If you have any questions regarding your results, please call (205) 975-8615.

Prescriptions
To obtain a refill for any medications prescribed by this clinic, please call (205) 975-8615.

Patients with Limitations
Patients visiting The Kirklin Clinic may take advantage of convenient parking in the deck adjacent to the Clinic. Located at 2100 6th Avenue South, the parking deck is connected to the second floor of The Kirklin Clinic by a weatherproof sky bridge. High top van access is restricted to the first floor of the clinic parking deck. Therefore, patients arriving in high top vans and requiring wheelchair assistance should be dropped off at the front door of the clinic before the driver parks the vehicle. Valet parking is available on Level 2 of the deck; wheelchairs and additional assistance are located here as well.
